Obtaining a License
LOST/ EXPIRATION (OUT OF STATE)
1.Requirements
- Identification: Birth certificate/ citizenship certificate of registration/ permanent residency/ passport (that can prove legal stay)
- Proof of residence: (refer to ‘new driver’s license’)
- Social security card
- If applicant lost the out of state license, he needs to get invalidity confirmation of driver’s license or driving record from final issuing organization of driving license
2.Commission: $20 (5 years), $35 (10 years)
